Internally connecting with the five zang organs, spreading through the intestines and stomach, affecting both yin and yang, thus injuring the five zang. This is how pathogenic influences, entering from the skin and hair, progress inward to affect the five zang in sequence. In such cases, treatment should be directed at the meridians. Now, when pathogenic influences dwell in the skin and hair and enter into the fine collaterals, if they remain without leaving, blocking and obstructing passage, they cannot penetrate into the meridians but instead overflow into the major collaterals, giving rise to unusual diseases. When pathogenic influences affect the major collaterals, they may move from left to right or from right to left. They spread upward and downward, left and right, intersecting with the meridians, and distribute toward the four extremities. Their qi has no fixed location and does not enter through the regular shu points; this is called miu ci. The Emperor said: I wish to hear about miu ci—treating the right side for a condition on the left, and treating the left side for a condition on the right. How is this done? How does it differ from ju ci? Qibo said: When pathogenic influences dwell in the meridians, if the left side is excessive, it causes illness on the right; if the right side is excessive, it results in illness on the left. There are also cases where this shifts—pain begins on the left but before it subsides, the pulse on the right becomes affected first. In such situations, ju ci must be applied, and needling must precisely strike the meridian involved; these are not collateral vessels. Therefore, when a disease affects the collaterals, its pain is located in a position that contradicts the course of the meridians; thus it is called miu ci.
