孝宗成穆郭皇后，开封祥符人。奉直大夫直卿之女孙，其六世祖为章穆皇后外家。孝宗为普安郡王时纳郭氏，封咸宁郡夫人。生光守及庄文太子、魏惠宪王恺、邵悼肃王恪。绍兴二十六年薨，年三十一，追封淑国夫人。三十一年，用明堂恩，赠福国夫人。既建太子，追封皇太子妃。及受禅，追册为皇后，谥恭怀，寻改安穆。及营阜陵，又改成穆，祔孝宗庙。
